Some minor mathematical errors in the descriptions of the explicit gate
actions in :gate:`sdg`, :gate:`rx`, :gate:`ry`, :gate:`rz`, :gate:`crx` and
:gate:`cry` were corrected. The gates have not changed their intended
meanings.
