Rename diagnostic derive things.

For increased consistency.

- session_diagnostic_derive    -> diagnostic_derive
- session_subdiagnostic_derive -> subdiagnostic_derive
- SubdiagnosticDeriveBuilder   -> SubdiagnosticDerive
